BOUNDS Portal offers an easy to use solution for the state to receive incident reports
from residential care and other providers. The reports are received in a timely, consistent manner which creates visibility for all state programs that need information from the facilities. Integrated cross reporting is one of the features most requested by programs cooperating between licensed facility programs for vulnerable adults, Ombudsman, and Adult Protective Service.
Consistent Cross Reports provide benefits for residents. Facility residents benefit from the automated submissions of the incidents to the State Long Term Care Ombudsman (for Long Term Care and Residential Assisted Living Facilities) where the reports are reviewed to determine if Ombudsman involvement is needed as resident advocates.
Additionally, incidents are cross reported to Adult Protective Services, the program that investigates reports of abuse, neglect, and exploitation. The facility doesn’t have to remember to cross report or take extra steps to complete these notifications as the system does it automatically. APS reviews the submissions to determine if a report meets APS criteria for involvement or not.
