Across the country, rural communities are working to expand access to essential healthcare services and infrastructure. The Rural Health Transformation Program (RHTP) is a five-year funding initiative that enables states to strengthen workforce shortages, foster greater collaboration between patients and providers and direct resources to areas with the greatest needs. By supporting innovation and connectivity, RHTP empowers communities to build more resilient, accessible and effective healthcare systems.
At the 2026 Rural Health Transformation Summit, State leaders discussed opportunities, priorities and what constituents can expect in the first two years of the program’s implementation. Sponsors of the event, Oracle and InterSystems, exhibited their technology at the Summit, demonstrating how collaboration and modernization efforts can benefit local communities. The following insights emerged as panelists, from State administrators to Private Sector experts to medical professionals, discussed how officials can successfully implement RHTP in their states.
Policy and Strategy Alignment is Critical
The goal of RHTP is to create a blueprint able to support small health organizations and connect them with larger organizations, regardless of funding levels. Policy and strategy alignment across State agencies is necessary to build long-lasting systems to support the most underserved/vulnerable population. Across the Summit, panelists agreed that healthcare solutions must be exclusively constructed for rural users rather than adapted from urban models. Critical to this strategy is making the definition of “rural” consistent across State agencies.
Healthcare Systems Thrive Through Collaboration
To be a nationwide success, RHTP requires collaboration between agencies and across State Government, including Medicaid programs, workforce efforts and broader policy priorities. Information must flow freely in order to properly assess technology gaps, leverage funding and address workforce shortages. Aligning rural health investments with broader State strategies ensures the projects’ longevity past RHTP funding.
Additionally, expanding data sharing across state lines can help create a more connected healthcare experience. Many rural patients travel to nearby metropolitan areas to access specialized care, and improved interoperability can ensure providers have seamless access to patient information, supporting more coordinated, efficient and personalized treatment. For example, patients in rural Delaware travel to cities across state lines, like Philadelphia, Baltimore and New York City. States utilize different methods to store, move and exchange patient data. Formalized collaboration and data exchange between providers, supported by unified data platforms such as MongoDB, should serve as the backbone of State RHTP plans.
Telehealth Adoption is Key
Telehealth is a fundamental aspect of the modern healthcare landscape, especially in areas where constituents are more geographically spread out. In RHTP, telehealth policy is focused on five major areas:
- Centralized hub models
- Mobile health
- Specialty Access
- Remote patient monitoring/at home care
- Artificial intelligence (AI)-enabled workflow integration
As states allocate investments to telehealth, special attention should be paid to sustainability planning and appropriate safeguards to secure Protected Health Information (PHI). Additionally, these solutions, including AI-enabled platforms such as Google Cloud, should fit into provider workflows and reduce barriers to purchasing and implementation, particularly in low-broadband environments.
The Rural Technology Catalyst Fund (RTCF) is a grant-based program for health-focused companies focused on rural care improvement. With eight states already participating, the RCTF is a valuable resource for states looking to integrate telehealth and other technology solutions, including drones and AI.
Potential Vendors Must Recognize RHTP Short- and Long-Term Goals
States recognize the importance of bolstering services and response times to meet program needs. Speed of adoption is a major priority, with most of the momentum focused on addressing workforce challenges. Simultaneously, without proper resources, infrastructure and planning, RHTP projects will not survive past the five-year point. Focusing on evidence-based programs that can start, stop and adapt as needed, rather than proof-of-concept programs, will meet those specific needs.
States are looking to partner with vendors that can rapidly move funding to rural communities while supporting durable healthcare programs. Accordingly, vendors that have established relationships with local communities and understand the rural healthcare landscape will be better positioned as states shift their focus from planning to execution.
Year One Focuses on Infrastructure Set-Up
During the first year of RHTP, states are focusing on setting up administrative frameworks and process development, with content management solutions such as Laserfiche helping to streamline the growing volume of paperwork. More people than ever before are now involved in the grant process, increasing the need for improving cross-agency collaboration. Funding is a primary focus in year one, with states looking to retain more funding locally to sustain RHTP past the five-year funding period.
States should expect to see significant transformation in year two of RHTP. In 2027, program initiatives will focus on working with rural providers, including installing alternative payment models, addressing workforce challenges and support for rural providers entering risk-based arrangements.
RHTP is both a boon and a test for State administrations. Over the next five years, states will need to balance rapid adoption with long-term sustainability to ensure projects during this period do not disappear once RHTP ends.
